There is no form on this page on purpose. We do not collect visitor information, and a site that takes your details is a different kind of site than this one.
With that we can usually correct a page the same day and roll the verification date forward.
If you are the office that sets these fees and something here is out of date — or you would rather we linked a different document — email us and we will make the change. If you publish a fee schedule that is hard to find or is a scanned image, tell us where the readable version lives and we will point at that instead.
We cannot tell you whether to file a case, how to fill out a form, what your chances are, or what a judge will do. We are not lawyers. For procedural questions, the Illinois Courts self-help center is the right starting point, and Illinois Legal Aid Online has free guided help. If you are facing an eviction, contacting legal aid early matters more than anything on this site.
